Wie an jedem anderen Körperteil können sich auch an der Hand Tumore entwickeln, die gutartig oder auch bösartig sein können. Meist ist ein operativer Eingriff zur Tumorentfernung notwendig.
Bei gutartigen Wucherungen besteht manchmal eine Erbkrankheit oder eine Häufung in der Familie, die Ursache von bösartigen Tumoren kann meistens nicht festgestellt werden. Schädigende Einflüsse können unter anderem Strahlung oder langwierige Entzündungen sein. Manchmal handelt es sich bei einem Handtumor auch um eine Metastase (Tochtergeschwulst), die sich aus einem anderen Körperteil abgesiedelt hat.
An der Hand kommen Weichteiltumore und Knochen- und Knorpeltumore vor.
Handtumore können gutartig oder, wesentlich seltener, bösartig sein. Sie können auf die Haut begrenzt sein, im Gewebe darunter zu ertasten und deutlich zu sehen sein oder aber sich im Inneren unbemerkt entwickeln, beispielsweise Knochentumoren. Neben der umschriebenen Verdickung bestehen gelegentlich Schmerzen. Bösartige Befunde können das umliegende Gewebe (z.B. auch Knochen) zerstören und dadurch Beschwerden verursachen. Ebenfalls können sie Tochtergeschwülste (Metastasen) an anderer Stelle bilden und dort symptomatisch werden.
Zur Diagnostik gehört zunächst die Anamnese (Patientenbefragung) und die körperliche Untersuchung. Bildgebende Verfahren (Röntgen, Ultraschall, CT = Computertomographie) werden angewendet, um Lage, Ausdehnung und Begrenzung des Tumors festzustellen. Die feingewebliche Untersuchung (Histologie), die meist erst nach der Operation am Handtumor vorgenommen wird, erbringt den definitiven Befund.
Die verschiedenen Arten von Tumoren müssen unterschieden werden. Außerdem muss bei einer schmerzhaften Schwellung auch an einen Abszess (abgekapselte Entzündung) oder eine andere Entzündung gedacht werden.
Vor der Operation eines bösartigen Tumors wird oftmals eine Strahlen- oder Chemotherapie durchgeführt, um die Ausdehnung zu verringern und den Eingriff somit zu vereinfachen sowie den Verlust an umgebendem, funktionsfähigem Gewebe gering zu halten.
Auch Tumoren, bei denen angenommen wird, dass sie gutartig sind, sollten in der Regel herausoperiert werden, da sie sich ebenfalls weiter ausdehnen und Probleme bereiten können.
Die Handtumor-Operation erfolgt in örtlicher Betäubung, Regionalanästhesie (Betäubung eines größeren Körperbereiches, hier des Armes) oder in Vollnarkose.
Es kann eine so genannte Blutleere zur Operation vorgenommen werden. Dazu wird eine Manschette um den Arm gelegt, um den Blutfluss zu stoppen. Ermöglicht wird somit eine bessere Sicht, da weniger störendes Blut vorhanden ist, und außerdem ist der Verlust an Blut geringer.
Der Tumor wird komplett herausgeschnitten. Dazu muss ein Sicherheitsabstand eingehalten werden, damit möglichst keine Reste verbleiben. Der Operateur achtet darauf, dass Strukturen wie Blutgefäße, Nervenstränge und Sehnen erhalten bleiben, so weit es geht. Besonders bei bösartigen Tumoren ist dies oftmals nicht möglich, so dass eventuell die Hand oder Anteile davon weggenommen werden müssen, um einer Ausdehnung und Streuung der Geschwulst vorzubeugen.
Im Anschluss wird häufig ein Verband mit stabilisierender Schiene, z.B. aus Gips, angelegt.
Kann ein größeres Stück Gewebe, z.B. Haut oder Knochen, nicht erhalten werden, so kann oft körpereigenes Gewebe aus der Umgebung oder von anderer Stelle verpflanzt oder umgesetzt werden. Beispielsweise ist die Verpflanzung eines Knochenstückchens, meist aus dem Beckenkamm, bei gewissen Voraussetzungen möglich.
Strukturen im Operationsgebiet können verletzt werden. Es können Blutungen, Nachblutungen und Blutergüsse (Hämatome) entstehen, bei Nervenverletzungen kann es zu Taubheitsgefühl oder Lähmungserscheinungen kommen, was nur selten dauerhaft ist. Infektionen, Wundheilungsstörungen sowie überschießende Narbenbildung können sich ergeben. Auch allergische Reaktionen können auftreten. Bei angelegtem Verband können Knochen und Weichteile stark geschwächt werden, auch das Sudeck-Syndrom, bei dem zusätzlich starke Schmerzen bestehen, kann nicht ausgeschlossen werden.

In den allermeisten Fällen kann der Tumor durch den Eingriff erfolgreich entfernt werden. Sollte sich jedoch in der feingeweblichen Untersuchung zeigen, dass noch Restgewebe vorhanden ist, muss eine weitere Operation oder gegebenenfalls auch eine Arzneimittelgabe erfolgen.
Beeinträchtigungen an Strukturen wie Sehnen oder Gelenken können oft nicht wieder rückgängig gemacht werden. In diesem Fall können zusätzliche Therapiemethoden, beispielsweise Krankengymnastik oder Elektrotherapie, angezeigt sein.
Oftmals müssen gerinnungshemmende Medikamente wie Aspirin® oder Marcumar® abgesetzt werden. Dies erfolgt in Absprache mit dem Arzt.
Bei ambulant stattfindender Operation muss sich der Patient abholen lassen und sollte für einen Tag kein Auto fahren, keine Maschinen bedienen und auch keine bedeutsamen Entscheidungen treffen.
Für eine Zeit lang ist eine Schonung der operierten Hand notwendig. Krankengymnastik kann den Heilungsverlauf positiv beeinflussen, insbesondere müssen Gelenke, die von der Operation nicht betroffen sind, regelmäßig bewegt werden.
Zeigen sich Besonderheiten, die Symptome einer Komplikation sein könnten, sollte der Arzt kurzfristig informiert werden.
